Sculptra is an injectable collagen stimulator containing poly-L-lactic acid. It does not behave like hyaluronic-acid filler and does not create its final result immediately.
Sculptra's recognised indications and permitted treatment areas vary between jurisdictions. Current US product information covers specified facial wrinkles and folds; buttock treatment is outside that facial indication and must be supported by the applicable manufacturer instructions, practitioner training, clinical assessment and insurance in the country where treatment is performed.

What is normal after treatment?
Normal
During the initial recovery period, it may be normal to experience:

Sculptra's official safety information lists swelling, tenderness, redness, pain, bruising, bleeding, itching and lumps among possible injection-site effects. Contact Layali Clinic if symptoms are severe, worsening or not improving as expected.
The immediate fullness is not the final result
Immediately after treatment, the area may appear fuller because of:
- Sterile water used to prepare the product
- Local anaesthetic
- Swelling
- Temporary tissue expansion
This initial fullness normally reduces as the fluid is absorbed. You may therefore notice that the area appears less full several days after treatment. This does not mean that the treatment has failed. Sculptra works by stimulating a gradual tissue response. Visible improvement develops over the following weeks and months. Facial clinical-study participants have experienced results beginning from approximately one month, sustained for up to two years in some studies. Buttock results are less predictable. Results vary and cannot be guaranteed.
The 5–5–5 massage routine

This routine helps distribute Sculptra evenly and may be modified by your practitioner.
Unless your practitioner has given different instructions, follow the 5–5–5 rule: massage the treated area for 5 minutes, 5 times each day, for 5 consecutive days. The manufacturer's patient guidance recommends this routine to help distribute Sculptra evenly. Your practitioner may extend, shorten or modify the routine according to treatment area, number of vials, injection plane, tissue response, swelling or bruising and individual findings. Follow the personalised instructions recorded on your treatment plan.
How to massage safely
Before beginning
- Wash your hands thoroughly
- Ensure the entry points are not actively bleeding
- Use clean skin
- Use a small amount of gentle moisturiser or massage cream if advised
- Position yourself comfortably
- Follow the direction demonstrated by your practitioner
During massage
- Use the flat pads of your fingers or palm
- Apply firm but comfortable pressure
- Massage the entire treated area evenly
- Use controlled circular and sweeping movements
- Include the edges of the treated area as demonstrated
- Do not focus repeatedly on only one lump or point
- Complete the full advised massage time
Massage may feel mildly tender, particularly during the first few days, but it should not cause severe pain.

Manual massage should be used unless your practitioner has specifically approved another method.
When not to massage

Do not massage directly over an open entry point, broken skin, a suspected infection, a blister, an area of suspected tissue damage, or a very painful or rapidly enlarging swelling.

- Drink water normally
- Use a clean, wrapped cool pack if recommended

- Use excessive massage pressure
- Apply ice directly to the skin
- Use heat packs
- Attempt to squeeze out or reposition the product
- Allow another practitioner or massage therapist to manipulate the area without approval
Cold compresses
A clean cool compress may be used if advised.
When using a cool compress
- Wrap the cool pack in a clean cloth
- Apply it briefly
- Do not apply firm pressure
- Allow breaks between applications
- Do not apply frozen products directly to the skin
- Do not fall asleep with a cooling pack in place
Cooling does not replace the prescribed massage routine.
Washing and hygiene
You may cleanse or shower gently once your practitioner confirms the entry points can become wet. During early healing:

- Avoid scrubbing
- Avoid exfoliating gloves
- Pat the area dry
- Use clean towels
- Avoid fragranced products over entry points
- Avoid baths, swimming pools and hot tubs until entry points have closed
- Do not shave, wax or exfoliate directly over irritated entry points

- Becomes increasingly red, hot, swollen or painful
- Does not appear to be healing
Makeup and facial skincare
Official product information permits makeup several hours after treatment, but waiting until the injection points have closed and the following day is preferable. When restarting:

- Avoid heavily rubbing the skin
- Do not apply makeup over bleeding or open entry points
For approximately 24–48 hours, or until irritation settles, avoid:

Gentle massage performed as part of the instructed Sculptra routine is still required. Avoid all additional or aggressive facial manipulation.
Exercise
Avoid strenuous exercise for approximately 24–48 hours, or longer if the area remains significantly swollen, painful or bruised. This includes:

- Running
- Heavy weightlifting
- High-intensity interval training
- Hot yoga
- Vigorous cycling
- Contact sports
- Heavy glute training after buttock treatment
- Squats, lunges or hip thrusts that cause significant discomfort
- Exercises involving repeated pressure against the treated area
Gentle walking is generally acceptable. Return to exercise gradually.
Stop & contact the clinic if activity causes
- Increasing pain
- Sudden swelling
- Bleeding
- A new painful lump
- Skin-colour changes
- Significant asymmetry
- Breathing or neurological symptoms
Heat, sun and swimming
Until redness, swelling and tenderness have settled, avoid:

- Saunas
- Steam rooms
- Hot tubs
- Sunbeds
- Very hot baths
- Heated treatment devices
- Intense sun exposure
- Extreme cold exposure
Avoid swimming until the entry points have closed. Use broad-spectrum sunscreen on exposed facial areas once the skin can tolerate it. Do not expose bruised or inflamed skin to deliberate tanning.
Alcohol, smoking and medication
Alcohol: avoid for at least 24 hours and while significant bruising or swelling remains.
Smoking & nicotine: avoid or reduce smoking, vaping, shisha and other nicotine use during healing.
Medication:
- Do not stop prescribed medication without speaking to the clinician who prescribed it
- Do not stop prescribed blood thinners on the clinic's advice alone
- Do not take leftover antibiotics
- Do not apply unprescribed antibiotic or steroid creams
- Ask your practitioner, pharmacist or prescriber which pain relief is appropriate

- Avoid sleeping with strong pressure against the treated side during the first night where possible

- Do not repeatedly press the skin looking for lumps
- Allow swelling and bruising to settle before assessing symmetry

- Avoid tight jeans and restrictive shapewear during early healing
- Avoid firm pressure from belts, seams or tight underwear

- Avoid strenuous glute exercises for approximately 24–48 hours
- Change sitting position regularly if the area is tender
- Avoid prolonged pressure if it increases pain
- Sleep in a comfortable position that does not cause significant pressure or discomfort
Unlike surgical fat transfer, Sculptra does not automatically require a surgical BBL pillow or prolonged avoidance of sitting. Use one only if your practitioner has recommended it.

- Severe or increasing deep pain
- A hot, red or painful lump
- Fluid or pus from an entry point
- A soft fluid-filled swelling

Lumps, nodules and firmness
Temporary firmness or small areas of swelling may occur. Sculptra can also cause delayed nodules or lumps. These may:
- Be felt beneath the skin
- Be visible
- Be painless
- Become tender or inflamed
- Develop weeks, months or occasionally longer after treatment
- Require medical treatment
- Rarely require surgical treatment
Official safety information reports small palpable bumps and larger delayed lumps, with or without inflammation or skin discolouration. Long-term monitoring has documented injection-site nodules following facial Sculptra.
Contact if you develop
- A persistent lump
- A hard or enlarging nodule
- A painful lump
- A red or warm lump
- Multiple new lumps
- A visible contour irregularity
- Swelling that returns after settling
- A lump appearing after illness, infection, vaccination or dental treatment
Do not repeatedly squeeze, needle or aggressively massage a persistent nodule without clinical advice.
Sculptra cannot be dissolved with hyaluronidase
Sculptra is made from poly-L-lactic acid. It is NOT hyaluronic-acid filler. Hyaluronidase breaks down hyaluronic acid and is not an antidote or routine dissolving treatment for Sculptra.
Sculptra-related lumps or inflammatory reactions may require clinical observation, medical treatment, prescription medication, specialist assessment, imaging, intralesional treatment, drainage, or rarely surgical management. Do not arrange hyaluronidase treatment elsewhere without first informing the clinician that the product is Sculptra.
Delayed inflammatory reactions
Delayed inflammation may develop after the initial treatment has appeared to settle. Possible symptoms:
- New swelling
- Redness
- Tenderness
- Firmness
- Multiple lumps
- Recurrent swelling
- Warmth
- Skin discolouration
Updated product labelling recognises that delayed inflammation may follow viral or bacterial illness, infection, vaccination or dental procedures.
Contact if symptoms develop after

This does not mean every illness or vaccination will cause a reaction, but any new delayed swelling or nodules should be assessed.

Treatment course and results
Sculptra treatment usually involves a planned course rather than one session. Official facial guidance states many clients undergo approximately two to three sessions over several months, spaced at least three to four weeks apart. Do not have another session simply because the temporary fluid-related fullness has reduced.
