After Your Treatment

PDO Cog Thread Lift Aftercare & Recovery

For lifting threads placed in the cheeks, mid-face, jawline, jowls, brows and neck.

Please follow these instructions carefully after treatment. PDO cog threads are absorbable barbed sutures placed beneath the skin to reposition and support facial tissue. The threads require time to settle and become supported by the healing tissue.

The advice given by your practitioner for the exact thread brand, number of threads, treatment vectors and areas treated takes priority over this general guide.

Managing your symptoms at home

Tightness, a pulling feeling and mild dimpling are common early on — here's how to stay comfortable:

Helps

  • Sleep on your back with your head slightly raised for 5–7 nights
  • A wrapped cool compress applied gently
  • Soft foods; support your jaw gently when you yawn
  • Paracetamol if needed

Skip

  • Massage, pressure or rubbing
  • Big mouth-opening and hard/chewy foods for 1–2 weeks
  • Exercise for 7 days
  • Heat, saunas & swimming for ~1 week
Mild dimpling or a pulling feeling usually settles as the tissue relaxes. Contact us for a visible thread, severe or worsening dimpling, or spreading swelling.

What is normal after treatment?

Normal

During the initial recovery period, it may be normal to experience:

Completely normal

  • Swelling
  • Bruising
  • Redness
  • Tenderness or soreness
  • Tightness
  • A pulling or tugging sensation
  • Mild aching
  • Mild itching
  • Temporary numbness or altered sensation
  • Awareness of the threads
  • Temporary asymmetry caused by swelling
  • Mild skin puckering or dimpling
  • Temporary rippling
  • Small entry-point marks
  • Mild discomfort when chewing, smiling or opening the mouth
  • A clicking or pulling sensation with some facial movements
  • Minor firmness along the thread pathway

Swelling, bruising, pain, temporary dimpling and tissue irregularity are recognised early effects of facial thread lifting. Other reported complications include thread visibility, palpability, migration, extrusion, infection and nerve-related symptoms. The initial lift may appear stronger or less symmetrical than expected.

Do not judge the final result while swelling and tissue tension are still present. Contact Layali Clinic if a symptom is severe, worsening or different from what was explained during your consultation.

Immediately after treatment

Do

  • Rest for the remainder of the treatment day
  • Keep the entry points clean and dry
  • Wash your hands before touching near the treated area
  • Keep your head upright where possible
  • Use a clean, wrapped cool compress if advised
  • Take only the pain relief recommended as suitable for you
  • Eat small, soft meals if chewing feels uncomfortable
  • Keep the clinic's contact details available
  • Follow all personalised instructions given before leaving the clinic

Do not

  • Rub, press or massage the treated area
  • Pull or stretch the skin
  • Attempt to release dimpling yourself
  • Attempt to reposition the threads
  • Apply strong pressure with a cool pack
  • Apply ice directly to the skin
  • Use facial rollers, gua sha tools or massage devices
  • Pick at entry points or scabs
  • Allow another practitioner or therapist to manipulate the threads
  • Apply unprescribed creams or medication to the entry points
Manufacturer guidance supports using clean wrapped cold compresses for early swelling while avoiding direct ice contact and unnecessary pressure.

Entry-point care

Small needle or cannula entry points may be visible after treatment. For the first 24 hours, unless instructed otherwise:

For the first 24 hours

  • Keep the entry points clean and dry
  • Do not apply makeup over them
  • Do not repeatedly inspect or press them
  • Do not remove adhesive strips or dressings early
  • Do not apply oil, perfume, fake tan or fragranced products
  • Avoid hair products running onto entry points near the hairline
  • Do not pick at crusting or small scabs

Once your practitioner confirms that washing is permitted:

When washing is permitted

  • Wash your hands first
  • Cleanse gently with lukewarm water and a mild cleanser
  • Use light contact only
  • Pat the skin dry with a clean towel
  • Do not drag the skin downward
  • Avoid cleansing brushes and exfoliating cloths

Contact the clinic if an entry point

  • Continues bleeding
  • Opens
  • Begins leaking
  • Produces pus
  • Develops an unpleasant smell
  • Becomes increasingly red, hot, swollen or painful
  • Develops a blister or open wound
  • Has a visible thread emerging through it

Do not cut, pull or push back an exposed thread.

Cold compresses

Where recommended:

Do

  • Wrap the cool pack in a clean, soft cloth
  • Apply it gently without firm pressure
  • Use it intermittently rather than continuously
  • Avoid dragging it across the face
  • Stop if the skin becomes excessively numb or uncomfortable

Do not

  • Place ice directly on the skin
  • Press firmly over the thread pathways
  • Fall asleep with a cool pack in place
  • Use an unclean reusable pack against an entry point

Cold compresses are included in recognised thread-lift recovery guidance for managing early swelling, bruising and discomfort.

Sleeping position

For approximately the first five to seven nights, or for the period specified by your practitioner:

Sleeping guidance

  • Sleep on your back
  • Keep your head slightly elevated if comfortable
  • Use clean pillowcases
  • Avoid sleeping on your side
  • Avoid sleeping face-down
  • Avoid pressing the treated area against your hand, pillow or bedding
  • Consider placing pillows beside you to reduce accidental rolling

Do not use restrictive facial straps, tight chin supports or garments unless specifically approved. Contact the clinic if sleeping pressure causes sudden pain, a new indentation, thread movement or a change in facial shape.

Facial movement

Cog threads mechanically engage the tissue. Excessive facial movement during early healing may cause discomfort or place strain on the treated area. For approximately one to two weeks, depending on the treatment area and your recovery:

Avoid

  • Minimise exaggerated smiling
  • Avoid opening the mouth extremely wide
  • Support your jaw gently if you need to yawn
  • Avoid prolonged laughing that causes significant pulling
  • Avoid chewing gum
  • Avoid very hard or chewy foods
  • Avoid large bites
  • Avoid forceful facial exercises
  • Avoid repeatedly testing the lift in a mirror
  • Avoid stretching the neck forcefully after neck treatment

A peer-reviewed review recommends avoiding wide mouth opening, hard foods, strong pressure and facial massage for approximately one to two weeks. Normal gentle speaking and facial expression are acceptable. The goal is to avoid extreme movement, not to keep the face completely still.

Food and drink

If the cheeks, lower face or jawline were treated, use a soft diet for the first several days or as advised.

Suitable

  • Yoghurt
  • Soft eggs
  • Soup that is not excessively hot
  • Mashed foods
  • Soft pasta
  • Rice
  • Soft fish
  • Smooth foods that require minimal chewing

Avoid temporarily

  • Tough meat
  • Hard bread
  • Large sandwiches
  • Chewing gum
  • Crunchy foods
  • Very large bites
  • Foods requiring prolonged chewing

Wait until normal sensation has returned before consuming very hot food or drinks, so you do not accidentally burn yourself. Drink normally and remain appropriately hydrated.

Dental treatment

Routine elective dental treatment can require prolonged wide mouth opening and may place pressure on the cheeks or jaw. Inform your dentist that you have recently had facial lifting threads. Where clinically reasonable:

Where clinically reasonable

  • Postpone elective dental treatment during the early healing period
  • Ask Layali Clinic when it is appropriate to proceed
  • Avoid routine dental work for approximately two to four weeks when facial or jawline threads have been placed, unless your dentist considers treatment medically necessary
  • Do not postpone urgent treatment for dental infection or severe dental pain

Contact if you develop

  • Tooth infection
  • Dental abscess
  • Facial swelling
  • Gum infection
  • A need for emergency dental treatment

Medical or urgent dental care must take priority. Tell the dentist the location, date and type of threads used.

Exercise

Avoid strenuous exercise for at least seven days, or longer if advised because of extensive treatment, swelling or discomfort.

Avoid

  • Running
  • Heavy weightlifting
  • High-intensity interval training
  • Hot yoga
  • Contact sport
  • Inverted yoga positions
  • Vigorous dancing
  • Activities involving tight helmets
  • Activities with a risk of facial impact
  • Heavy lifting that causes straining
  • Forceful neck exercises after neck threads

Gentle walking is generally suitable if you feel well. Return to exercise gradually.

Stop & contact if exercise causes

  • Increasing pain
  • Sudden swelling
  • Bleeding
  • A new indentation
  • Thread movement
  • A visible thread
  • Significant facial asymmetry

Heat, swimming and sun exposure

For approximately the first week, or until swelling and entry-point irritation have settled, avoid:

Please avoid

  • Saunas
  • Steam rooms
  • Hot tubs
  • Sunbeds
  • Very hot baths
  • Very hot showers
  • Heated facial devices
  • Intense sun exposure
  • Swimming pools
  • Sea swimming
  • Ice baths
  • Extreme cold exposure

Once the entry points have closed, use broad-spectrum sunscreen, avoid deliberate tanning over bruised or irritated skin, and wear a clean hat carefully, ensuring it does not press on entry points or thread pathways.

Makeup and skincare

Avoid applying makeup until the entry points have closed, ideally for at least 24 hours or for the period advised. When restarting, wash your hands first, use clean brushes and sponges, apply products gently, avoid dragging the skin downward, and do not cover an open, bleeding or infected entry point.

For the initial recovery period, avoid:

Avoid

  • Facial scrubs
  • Cleansing brushes
  • Chemical exfoliants
  • Retinol and tretinoin over irritated skin
  • Strong acids
  • Strong vitamin C over broken skin
  • Waxing
  • Threading
  • Dermaplaning
  • Home microneedling
  • Aggressive facial devices
  • Strongly fragranced products

Use gentle cleanser, moisturiser and sun protection when tolerated.

Massage and facial treatments

Do not massage or manipulate the treated area unless your practitioner specifically instructs you to do so.

Avoid

  • Facial massage
  • Deep cleansing facials
  • Gua sha
  • Facial rollers
  • Lymphatic facial massage
  • Buccal massage
  • Intraoral massage
  • Microcurrent devices
  • Massage guns
  • Cupping
  • Face-sculpting tools
  • Chiropractic or manual pressure over the treated face or neck
  • Massage treatments requiring you to lie face-down

Facial massage and vigorous facials may disturb healing thread placement. Do not attempt to massage out dimpling yourself — some irregularities require practitioner assessment and incorrect manipulation may worsen the result.

Alcohol, smoking and medication

Alcohol: avoid alcohol for approximately one week where possible, particularly while bruising and swelling remain.

Smoking & nicotine — avoid or reduce:

Avoid or reduce

  • Cigarettes
  • Vaping
  • Shisha
  • Nicotine pouches
  • Other nicotine products

Smoking and nicotine may impair normal wound healing.

Medication:

Severe, increasing or disproportionate pain must not simply be covered with pain relief. Contact Layali Clinic.

Dimpling, puckering and rippling

Mild dimpling or puckering can occur because the cogs engage the tissue and the surrounding area is swollen or under temporary tension. Some mild irregularities improve gradually as:

Improves gradually as

  • Swelling decreases
  • The tissue settles
  • Normal gentle facial movement resumes
  • The threads integrate with the surrounding tissue

Contact promptly if

  • Dimpling is severe
  • The skin appears tightly tethered
  • A depression is worsening
  • A new depression appears after several days
  • The area is painful
  • The skin changes colour
  • The irregularity is associated with a visible or palpable thread
  • The irregularity is not improving

Dimpling and contour irregularity are among the most frequently described thread-lift complications. Early assessment can be important because persistent tethering may become harder to correct once fibrosis develops. Do not attempt to release the dimpling yourself.

Thread awareness and pulling sensations

You may temporarily notice:

Temporarily notice

  • Tightness when smiling
  • Pulling while chewing
  • A clicking or catching sensation
  • Awareness of a thread path
  • Tenderness when turning the head
  • Mild thread palpability beneath the skin

Contact if

  • The sensation is severe or increasing
  • A thread feels sharply painful
  • You can clearly see the thread
  • A thread appears close to the skin surface
  • The thread appears to have moved
  • You develop a new linear depression or raised line
  • Movement suddenly changes the facial result

Do not repeatedly press the thread to check its position.

Thread protrusion or extrusion

Contact us

A thread emerging through the skin is not normal and requires prompt assessment. Contact Layali Clinic immediately if:

Contact Layali Clinic immediately if

  • A thread end becomes visible
  • A thread can be felt breaking through the skin
  • A small white or clear strand appears at an entry point
  • The skin opens over the thread
  • A thread protrudes inside the mouth
  • There is discharge or redness around a visible thread

Do not

  • Pull the thread
  • Cut the thread
  • Push it back under the skin
  • Cover it with makeup
  • Allow another person to remove it
  • Apply glue or adhesive over it

Thread extrusion may require thread removal and can be associated with infection or incorrect thread position.

Infection warning signs

Contact us

Contact Layali Clinic promptly if you develop:

Signs of infection

  • Increasing redness
  • Increasing heat
  • Worsening swelling
  • Throbbing or increasing pain
  • Pus or discharge

  • An open or non-healing entry point
  • A painful lump
  • Fever
  • Chills
  • Feeling generally unwell
  • Red streaks
  • Symptoms that improve and then worsen
  • Recurrent swelling along a thread pathway

Infection is a recognised thread-lift complication and can lead to abscess, chronic inflammation, granuloma, thread extrusion or the need for removal. Do not squeeze the area, attempt to drain it, insert a needle, pull an exposed thread, apply leftover antibiotics or delay contacting the clinic.

Bleeding and haematoma

Urgent

Some bruising is expected, but contact Layali Clinic urgently if you develop

  • Rapidly increasing swelling
  • A very tense or painful swelling
  • Rapidly spreading bruising
  • Continued bleeding
