• Photographs of the treatment area
• Your consent and signature

Why we use it & our lawful basis

We use your information to assess your suitability, plan and deliver your treatment safely, keep an accurate clinical record, and contact you about your care. Our lawful bases under UK GDPR are:

• Contract — to provide the treatment you have requested (Article 6(1)(b)).
• Explicit consent and the provision of health care for your health information and photographs (Article 9(2)(a) and 9(2)(h)).

We will only use photographs for marketing if you give separate, specific consent — and you can withdraw that at any time.

How we store & protect it

Your completed forms are transmitted securely and stored as part of your confidential clinical record, accessible only to those involved in your care. We take appropriate measures to keep your information safe.

How long we keep it

We keep clinical records securely in line with professional medical record-keeping guidance — typically for a minimum of 8 years after your last treatment (longer for patients under 18). (Husna — confirm your exact retention period with your insurer/governing body and update this line.)

Who we share it with

We do not sell your data. We only share it where necessary for your care or where required by law. Your forms are delivered to us using a secure third-party form service that processes the submission on our behalf.

Your rights

• Access a copy of your information
• Have inaccurate information corrected
• Request erasure (where there is no legal reason for us to keep it)
• Withdraw consent (for example, for marketing use of your photographs)
• Complain to the Information Commissioner's Office (ICO) at ico.org.uk
